Privacy Policy

THIS NOTICE DESCRIBES HOW INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

The Sullivan Center uses information collected in the aggregate, not individually, to collect statistical information for funding and administrative purposes. The individual does not have to give consent for use of information in this way. We do not disclose nonpublic personal information about staff, clients or volunteers, except as permitted or required by law.

The Sullivan Center only collects information it needs to determine whether it can provide assistance and the best assistance possible. Other uses or disclosures will be made only with the individual’s written authorization and that the individual may revoke such authorization.

Information provided by the individual will be shared only with the written permission of the individual and that individual may revoke such authorization at any time.

An individual has the right to see their personal information. This can be obtained by putting the request in writing or requesting an interview with the Executive Director. Every effort will be made to schedule the appointment within 48 hours.

The Sullivan Center is required by law to maintain the privacy of protected personal information and to provide individuals with notice of its legal duties and privacy practices with respect to protected personal information. We maintain physical, electronic, and procedural safeguards that comply with basic standards to guard information about you.

The Sullivan Center and its staff must adhere to strict policies and procedures to prevent any misuse of your nonpublic personal information. Employees are bound by this privacy policy and are educated on implementing security principles and practices.

The Sullivan Center reserves the right to change the terms of this notice and to make the new notice provisions effective for all protected personal information maintained. We will post any revisions and give written copies of the changes to all staff.

Individuals may file a complaint with The Sullivan Center if they believe their privacy rights have been violated. This complaint should be in writing and directed to either the Executive Director or President of the Board of Directors.

The individual will not be retaliated against for filing a complaint.
